Pharmacogenetic (PGx) Testing

Personalized Medication Selection Through Pharmacogenetics

At Aspire Medical Group, we offer pharmacogenetic (PGx) testing for selected patients when clinically appropriate. Pharmacogenetics is the study of how your genes may influence the way your body processes certain medications and your likelihood of experiencing medication side effects.

PGx testing can provide additional information that may help guide medication selection and dosing for certain psychiatric medications. However, genetic testing is only one component of the decision-making process and should always be interpreted alongside your symptoms, medical history, previous medication response, current medications, and overall clinical picture.

Our goal is to use every available tool to help develop the safest and most effective individualized treatment plan.

What Is Pharmacogenetic Testing?

Pharmacogenetic testing analyzes specific genes involved in medication metabolism and response. Some people process medications more quickly or more slowly than others due to inherited genetic differences. These variations may influence how certain medications work or increase the likelihood of side effects.

The results help identify whether certain medications may require dose adjustments or closer monitoring based on your individual genetic profile.

Genetic testing does not diagnose medical or psychiatric conditions and cannot predict which medication will work best for every patient. Instead, it provides additional clinical information that may support individualized prescribing decisions.

Who May Benefit?

Pharmacogenetic testing may be considered for patients who:

  • Have experienced significant side effects from psychiatric medications
  • Have had a poor response to one or more medications
  • Have required multiple medication trials without adequate symptom improvement
  • Have complex medication regimens or potential drug interactions
  • Are starting medications for which pharmacogenetic information may be clinically useful

PGx testing is not routinely recommended for every patient and is generally reserved for situations where the results are likely to influence treatment decisions.

What Medications Can Be Evaluated?

Depending on the testing panel, pharmacogenetic testing may provide clinically useful information for certain:

  • Antidepressants
  • Antipsychotic medications
  • Mood stabilizers
  • ADHD medications
  • Anti-anxiety medications
  • Certain pain medications

Not every medication has established pharmacogenetic recommendations, and not every genetic finding requires a change in treatment.

What to Expect

Testing is simple, non-invasive, and typically performed using a cheek (buccal) swab collected during your appointment.

Once your sample is received by the laboratory, results are typically available within several weeks. Your provider will carefully review your results with you and explain how they may—or may not—affect your treatment plan.

Medication decisions are always based on the complete clinical picture rather than genetic testing results alone.

Insurance Coverage & Cost

Insurance coverage for pharmacogenetic testing varies by insurance plan and clinical indication.

When appropriate, Aspire Medical Group will submit any required documentation to determine insurance eligibility. However, insurance coverage decisions are made solely by your insurance company, and we cannot guarantee coverage or override an insurance company's determination.

If testing is not covered by your insurance plan, self-pay options may be available. We will discuss anticipated costs with you before testing is ordered whenever possible.

Our Approach to Personalized Medicine

At Aspire Medical Group, pharmacogenetic testing is used as one component of an individualized, evidence-based treatment approach. While genetic information can provide valuable insights, medication selection is always based on your symptoms, diagnosis, medical history, previous treatment response, current medications, laboratory findings when appropriate, and personal treatment goals.

Our goal is to help reduce unnecessary trial-and-error prescribing while developing the safest and most effective treatment plan for each patient.

Aspire Medical Group utilizes Tempus® Pharmacogenetic Testing when clinically appropriate.

  • Do you offer crisis service or walk-ins?

We do NOT offer walk-ins or crisis services. If you have an emergency, DO NOT CALL THE OFFICE, CALL 911 INSTEAD. We recommend you visit an Emergency Room for any emergent or crisis situations, as they do not require an appointment and will take walk-ins and provide crisis services.

There are NO on-call providers or appointments at our offices after hours.

  • What are your fees?

Please confirm your insurance coverage with our office staff prior to scheduling an appointment. We do not provide free services. Telehealth appointments are still medical appointments and will be treated as such (including paying balances upon services rendered).

We offer assistance to individuals on a case-by-case basis. Payment plans, a sliding fee scale, and assistance with specialty medication discounts are available.

  • Will the cost of treatment be covered by my insurance plan?

We take a variety of insurances in MA and NH, as well as private pay (cash, debit, and all major credit cards). Your balance is due at your scheduled appointment (if the balance is not paid in full, the appointment will be rescheduled).

For more information, please call our office at 617-991-9151 or email us at info@aspiremedgroup.com

  • How long will my appointment last?

Initial psychiatric evaluation appointments are 45 minutes to 1 hour long.

Most follow-up sessions are 20-30 minutes, but they may be scheduled for longer to best serve the needs of the individual patient.

  • What will happen during my first appointment?

A review of your psychiatric history, presenting symptoms, current medications, and lifestyle will be discussed during the initial psychiatric evaluation. The provider will gather the required documentation to adequately assess, diagnose, and treat the patient.

The patient will be given treatment recommendations and discuss potential side effects, efficacy, and duration of treatment. The provider will develop a treatment plan that best suits the needs of the patient. The patient has the right to refuse treatment or inquire about alternative treatment options. If it is outside the scope of our practice or a higher level of care is warranted, we will provide referrals to a provider or facility that can better serve your needs.

  • How often do I have to follow up with my provider?

Most follow-ups occur between 1 week to 2 months. Rarely, 3-month appointments are scheduled for patients (usually only for established patients that have been stable on medication for a prolonged period of time).

The frequency of appointments varies depending on the complexity and case of the individual patient. Prescriptions are filled up until the next scheduled appointment. Patients are responsible for contacting the office if a refill is needed.

Please do not contact providers directly outside of office hours.

  • What is a PMHNP and what can they do for me?

A Psychiatric Mental Health Nurse Practitioner (PMHNP) is a certified advanced practice nurse that specializes in the diagnosis, treatment, and management of mental illness and psychological problems. Some providers also offer primary therapy services.

In this setting, the PMHNPs primary role would be to recommend, prescribe, and manage medications when appropriate and review the risks, benefits, and alternatives to medications. When appropriate, the PMHNP would order and review labs. PMHNPs primarily provide medication management and, occasionally, supportive therapy. Oftentimes, patients would also work with a therapist for therapy sessions. We have a relationship with several area therapists and can provide referrals to established patients.

  • Are Telehealth appointments available?

Our providers are available for in-person and telehealth consultations.

Online sessions allow you to see a provider from the safety of your home, at a time that is most convenient for you, through our easy scheduling platform.
